Title:  Forty Years in the Iron Mines: A research and operational retrospective
 
Description:  Abstract

In 1942, Fowler and Barer showed that blood donation caused a drop in hemoglobin, which recovered in 7 weeks, although 24% of donors did not recovered by 8 weeks. Hemoglobin recovery was increased 49% by administration of oral iron. Despite this, US blood banks allowed blood donations every 8 weeks and did not systematically recommend iron supplements to frequent donors. In 1981 Simon reported that frequent donors have high prevalence of iron depletion, that additional risk factors were female gender and active menstruation, and that risk was mitigated by self-administered iron. Despite physician and scientists’ concerns, increasing need for blood donations in the 1980’s and lack of clear health impacts from iron depletion caused these findings to have little impact on recruitment practices that emphasized frequent donations. Further, increasingly successful campaigns to collect blood in high school students, led to presentation of gallon pins at HS graduations.

Five developments have slowed this “iron mining” by blood centers. First, reduced demand for blood transfusions has decreased blood collection requirements. Second, the potential benefit of iron depletion for reducing atherosclerotic heart disease (the “Sullivan hypothesis”) has not been confirmed. Third, the role of iron depletion in lowering the hemoglobin of blood donors and the potential role of iron depletion in fatigue, pica, and restless leg syndrome have been recognized. Most alarming have been studies suggesting that neurocognition, and potential neural development is affected by iron depletion in young women. Fourth, consideration of the “Precautionary Principal” has been applied to iron depletion. Last, NHLBI has funded seminal studies over the past decade: RISE (REDS Donor Iron Status Evaluation), HEIRS (Hemoglobin and Iron Recovery Study), STRIDE (Strategies to Reduce Iron Deficiency), and CHILL (Comparison of the History of Donation and Iron Levels in Teen Blood Donors).
